Patients often ask where I see patients and where different types of nasal, sinus and rhinoplasty surgery are performed.
I currently work across three private locations in Hampshire:

Each site has a different role within my private practice.
Broadly:
Adnova Clinic is my main centre for cosmetic rhinoplasty, functional septorhinoplasty and revision rhinoplasty.
Medicana Winchester is used for private ENT, nasal and sinus assessment and for procedures such as septoplasty, turbinate surgery and sinus surgery.

The aim is to ensure that patients are seen in the most appropriate setting for the problem they actually have.

Adnova Clinic, Fareham: Rhinoplasty and Septorhinoplasty (nasal surgery)
Adnova Clinic is my principal location for self-pay rhinoplasty and septorhinoplasty surgery.

Medicana is therefore often the right starting point if your principal concern is breathing, sinus disease or another ENT problem, rather than a primarily cosmetic concern.
What if I need septorhinoplasty rather than septoplasty?
Some patients initially present with nasal obstruction but turn out to have a structural problem involving both the inside and outside of the nose.
Examples include:
significant post-traumatic deviation;
nasal valve collapse;
a crooked external nose;
loss of structural support; or
deformity that cannot be corrected adequately with septoplasty alone.
In those cases, I may recommend functional septorhinoplasty rather than isolated septoplasty.
Self-pay septorhinoplasty surgery is then performed at Adnova Clinic.
This distinction matters because septoplasty treats the internal septum, whereas septorhinoplasty can also address the external nasal framework and nasal valves.

Patients who live nearer Southampton, Eastleigh or Winchester may find an initial consultation at Nuffield Wessex more convenient.
However, self-pay rhinoplasty, septorhinoplasty and revision rhinoplasty surgery is performed at Adnova Clinic in Fareham. Your dedicated Adnova page also confirms that first consultations can be undertaken at Wessex Nuffield, with subsequent rhinoplasty care and surgery at Adnova. timbiggs.co.uk
This means the location of your first consultation does not necessarily determine the location of your operation.

The exact pathway depends on the diagnosis rather than just the symptom.

Rhinoplasty versus septoplasty: why the location may differ
Patients often use the terms septoplasty, septorhinoplasty and rhinoplasty interchangeably, but they describe different operations.
Septoplasty
Septoplasty treats the internal nasal septum.
It is usually performed to improve airflow caused by septal deviation.
Rhinoplasty
Rhinoplasty changes the external shape and structural framework of the nose.
Septorhinoplasty
Septorhinoplasty combines functional reconstruction with external nasal surgery.
This may be needed where nasal obstruction is caused by more than just the septum.
For example, patients with:
nasal valve collapse;
a crooked nose;
significant trauma;
external structural narrowing; or
collapse following previous surgery
may require a septorhinoplasty rather than a septoplasty.
This is one reason why a specialist assessment is useful before choosing a hospital or procedure.
What happens if I am not sure whether my problem is functional or cosmetic?
That is extremely common.
Patients may have:
a hump they dislike but also poor airflow;
a crooked nose following trauma;
a deviated septum with visible external deviation;
nasal valve collapse that affects both appearance and breathing;
an over-operated nose following previous surgery; or
concerns about both appearance and nasal function.
You do not need to diagnose the problem yourself before booking.
The consultation is designed to determine:
what is causing the symptoms;
whether the issue is internal, external or both;
whether treatment is medical or surgical;
whether septoplasty is sufficient;
whether functional septorhinoplasty is needed; and
which clinic and surgical setting is most appropriate.
Can breathing problems and cosmetic concerns be treated together?
Yes.
Where a patient has both functional and aesthetic concerns, these can often be treated during the same functional septorhinoplasty operation.
Examples include:
deviated septum plus crooked external nose;
nasal valve collapse plus a dorsal hump;
post-traumatic obstruction plus nasal asymmetry;
tip collapse with difficulty breathing; and
previous rhinoplasty with structural obstruction.
Combining treatment can be preferable to addressing the cosmetic and functional components as unrelated problems.

Where do I perform revision rhinoplasty?
My private revision rhinoplasty surgery is performed at Adnova Clinic in Fareham.
Revision surgery often requires more extensive reconstruction because previous surgery may have caused:
scar tissue;
loss of septal cartilage;
nasal valve collapse;
over-resection;
tip weakness;
dorsal collapse; or
asymmetry.
These cases may require:
septal cartilage;
ear cartilage;
donor rib cartilage;
fascia grafting;
Piezo bony correction; or
nasal valve reconstruction.

Piezo ultrasonic rhinoplasty
Piezo ultrasonic instrumentation allows precise cutting and reshaping of nasal bone.

Dorsal preservation rhinoplasty
Dorsal preservation aims to retain favourable native bridge anatomy while lowering or reshaping the nose in suitable patients.
These techniques are complementary but are not the same thing.

a GP referral;
pre-authorisation;
a membership number; or

Do I need a GP referral?
If you are self-funding, you can usually contact the relevant clinic directly without a GP referral.
If you are using private medical insurance, your insurer may require a referral or authorisation before your consultation.
The easiest way to check the correct pathway is to contact the relevant secretary.
