Is Nasal Packing Really Necessary After Rhinoplasty?

For many patients considering rhinoplasty, one of the most anxiety-inducing aspects is the idea of nasal packing, the thought of having the nose filled with gauze or sponges post-surgery can be off-putting. Fortunately, modern rhinoplasty techniques have evolved significantly, and in my practice, nasal packing is no longer necessary.


🛑 Why Nasal Packing Was Traditionally Used

Historically, nasal packing was used to:

  • Control bleeding after surgery

  • Support internal structures during early healing

  • Prevent septal hematomas


However, packing often caused significant discomfort, nasal obstruction, and anxiety, especially during removal. Although many surgeons still use packing methods.


Why I DO NOT Use Nasal Packing

Thanks to advances in surgical technique and perioperative care, I’ve eliminated the need for nasal packing altogether. Instead, I use a combination of methods that reduce bleeding and swelling at the source:

  • Cocaine and adrenaline: These agents constrict blood vessels and reduce intraoperative bleeding.

  • Tranexamic acid (TXA): A powerful antifibrinolytic that helps minimise postoperative bruising and bleeding.

  • Piezo ultrasonic instruments: Allow for precise bone reshaping with minimal trauma to surrounding tissues.

  • Dorsal preservation techniques: Maintain natural nasal structures and reduce surgical disruption.

  • Pre-injection protocols: Targeted local anaesthetic and vasoconstrictors reduce bleeding before incisions are made.


Together, these techniques result in a cleaner surgical field, less postoperative swelling, and a more comfortable recovery, without the need for packing.


😌 What Patients Can Expect Instead

After surgery, patients may experience:

  • Mild congestion due to internal swelling

  • A small external splint (removed at day 7) and dissolvable sutures

  • No painful packing removal


Most patients are pleasantly surprised by how manageable the recovery is, especially compared to what they may have read or heard about traditional rhinoplasty.
